According to the theory, Big Mom accelerates the development of her foetus in her womb by using her soul power to strengthen the foetus. This helps to speed up the process of pregnancy giving her a chance to get pregnant again and have more kids.
Big Mom gave birth annually for 42 years, beginning when she was 18 years old, 50 years ago. She had previously given birth to several children at once; the largest birth she had experienced was the birth of decuplets eighteen years ago. Oda has also stated that Big Mom is virtually always pregnant, thus this theory makes the most sense and answers the question perfectly.
