The stakes are at an all-time high with Homelander taking over Vought as CEO, Butcher separated from his team, and Hughie and Starlight dealing with their internal issues. Branded as the smartest person in the world, Sage becomes a close aide of Homelander and takes over many operations at Vought. One shot from the third episode of the season reveals a particularly horrifying truth that might lead to Homelander’s downfall.

Season 4 of The Boys introduced two new supes that were added to the Seven. While conspiracy theorist Firecracker uses her influence to turn the fans’ opinion in Homelander’s favor, Sister Sage has a much more interesting power. She is regarded as the smartest person in the world and she hatches strategies to let Vought be in control.

In the third episode of the series, Sister Sage is seen relaxing after a particularly tiring day with Homelander in her cabin. She is visited by The Deep, who Sage then seduces after feeding him an unhealthy treat. While the two get intimate, the camera moves towards a bloodied tool on a table. According to fans, Sage used the tool to lobotomize herself.

According to Web MD, Lobotomy is the process of severing connections from the frontal lobe to other parts of the brain. An X (formerly Twitter) user mentioned that Sage lobotomized herself to make herself less empathetic due to Homelander’s actions and not be easily swayed by emotions. To keep herself on track, she performed the painful surgery.

Though it is a painful experience, the supe is known to have moderate healing powers as mentioned in her file which is referred to in episode 1 of the season.

Actor Antony Starr’s Homelander is currently going through his worst ordeal yet. The antihero is balancing his image, taking over the company, and trying to raise a son in his own way. The emotions for the unhinged super are at an all-time high and he is susceptible to break at any time. The final scene in the third episode also indicates a breakdown on the horizon.

However, for now, he has Sister Sage and Firecracker to take care of Vought and his public image respectively. Firecracker seems to be motivated by her hate for Starlight as they had a ruffle during their pageant days, while Sister Sage seems to be strategically making herself the main brain behind the operations at Vought. She also replaced Ashley as CEO.

However, Homelander also seems to be facing a threat in the form of A-Train, who might finally be turning over to the good side. With Starlight and A-Train, who know the inner workings of Vought, threatening to expose them, Sister Sage’s lobotomy is the only thing keeping her from joining the other side.

But, if Sister Sage expresses her disdain towards Homelander and his ideas, she will be under threat from the unhinged hero. Hence, there are chances of her turning and joining Starlight’s ‘liberal’ crowd to avoid Homelander. With three Supes against him, Homelander might be facing his eventual downfall.
