Belonging to the Germa 66 royal family, he was considered a failure, but during the Wano arc, his powers kicked in. Sanji’s exoskeleton hardened and unlocked the ability to handle more heat and created his most potent technique, Ifrit Jambe. But there is a fan theory that claims him to have died and been replaced by his clone.

During the Whole Cake Island arc, he is forced into marriage with one of Big Mom’s daughters, Pudding, and has to leave Luffy and company. It was during that time he might have been killed or gone into a coma. His brothers might have beaten him to death, and Reiju awakened the clone to marry Pudding instead. During that time, Sanji was having flashbacks, which might be his memories of being transformed into the clone.

It can be explained by how Reiju reacted after Sanji was knocked out. She did not let the doctors touch him, the handcuffs were fake, and later, she felt guilty for her brother’s death. Reiju did not want the clone to suffer the same fate as her brother, so she wanted it to escape with Monkey D. Luffy.

Previously, Nami, Nico Robin, and Sanji had betrayed them, but it was for Luffy’s sake. It would not be the same if the clone is designed to betray them. It won’t feel sympathy and try everything in its power to hurt the entire crew.

Moreover, the sudden kicking of the genetic modifications shows that it might be Sanji’s clone. Since the clone was still new to the programs, it took some to get accustomed to all the settings. They would never imagine Vinsmoke Sanji to betray them, and it would break their morale. But it is still a fan theory. Eiichiro Oda loves to test everyone’s imagination. So, until he confirms it, there is no way of knowing whether Sanji is a clone.
